We are committed to providing accessible healthcare for people with learning disabilities.

We work with patients, families, carers, and support workers to make healthcare easier to access and understand.


Join Our Learning Disability Register

If you have a learning disability or think you may have a learning disability, please let us know. Mencap have some very useful information on what a learning disability is and how this differs to a learning difficulty.

Patients on our Learning Disability Register can benefit from:

  • An Annual Health Check from age 14
  • A personalised Health Action Plan
  • Reasonable adjustments to support access to healthcare
  • Additional support with appointments and communication

Annual health checks are recommended for everyone aged 14 and over who is on a GP Learning Disability Register.


Annual Health Checks

An Annual Health Check helps us to:

  • Identify health problems early
  • Review medicines
  • Discuss physical and mental health
  • Check hearing, vision, mobility, and wellbeing
  • Create a Health Action Plan

These checks can help improve health outcomes and ensure any concerns are identified as early as possible.

Reasonable Adjustments

We can discuss adjustments that may make accessing our services easier.

Examples include:

  • Longer appointments
  • Quiet waiting arrangements
  • Easy Read information
  • Support from a carer or advocate
  • Alternative communication methods

Carers and Support Workers

With the patient's consent, carers and support workers are welcome to attend appointments and help patients understand information and make decisions about their care.

We also want to provide help and support for carers where appropriate.
